A doctor

In sri lanka being a doctor is better than being a god. But If we discuss about this character there are many things a doctor must hold along. If we discuss about the stress a doctor take it is unimaginable. Since the first day of the medical college till the day graduation day a doctors mind and brain full of stresses. Exams, family, relationships, social isolation. and many.
Except all above mentioned things if you are in a foreign country the tress very high. Im not going to tell why, its better if you can experience it. After 6 year loneliness in a such a distant country after doing the studies to the maximum as we can we come to our own mother land to serve it.
My great wish is to serve my country. A doctor always should be ready to take any emergency till he get the emergency teems' assistance or till he get the specialist support. He should be a character have a ability to train his secondary staff. A doctor specially should have a keen knowledge and ability to manage  catastrophic events. which are with many casualties. A war, natural disaster and epidemic conditions are some examples.
When a country face to a conditions like epidemics a doctors character is very critical. Because he is the specialized person who comes to help our society. Sri lanka had gone through such epidemics recently. Except that he should be a person can be work as a family doctor, an emergency doctor a preventive medicine instructor even as a teacher. A teacher mean- a teacher to school teachers, nurses, lowest levels of health care personals.
He should be a kind character specially when  he works with pregnant mothers, children, and aged people.
He should be steady enough to see the unhappy ends of our patients. The doctor is doing his Maximum but the  out come is not good. prognosis is bad. Specially this happens with gynecologic,  and cardiac patients.
I would like to end the letter with a special extract,-
this was from the oxford clinical handbook of medicine.
the dedication was-the book is dedicated to those who are starting or in the thick of it, or to those who are at a sharp end of a difficult decision.

Witness of a abortion surgeries.

At the time Im writing this letter to you im not in a operation theater. Im in my university hostel in Perikalskawa.
Abortion is a discussed topic in our countries, According to the medical view, according to the social and religious view. Actually in Russia the main indications for a abortions are Fetal defects, Maternal factors which could be dangerous for both mother and the fetus. Except all these reasons the main reason for high abortion rate in Russia is mothers will.
Why mother's will plays a major role in abortion in Russia? There are some main reasons younger generation starts their sexual activities very early even before they have a sufficient education about sexual relationships, family planing and safety in sexual inter course. All above mentioned factors joined together not only with the higher rates of abortions but also higher incidents of sexually transmitted diseases. On this back ground education about these things in any country plays a major role. Fortunately in our countries the cultural background, religion mainly Buddhism plays a huge role. We should be thankful to these silent forces and workers of our community to keeping the society a clean and nonpolluting one. As a medical student im giving these credits to them. For me there is no big shame to tell about my cultural support in healthcare.
As medical students we know the procedure and the succession of a abortion. Actually this is a skill that we should know. We should be able to follow this procedure in an emergency. In sri lanka it is strictly in emergencies.

With the start of new semester we began our obstetrics and gynecology cycle. Last semester we have finished our Obstetrics part so we were appointed to the gynecology section of the hospital. We were taken to a operation there were ongoing three operations. Our teacher explained, all the surgeries are abortions. One Mother came to the room and she settled to the operation procedure a special anesthetic substance injected to the vagina and fornix. For the last time doctor asked from her "Have you changed your mind?" she was very silent. I saw only one tear came from her right eye. She was 18 and the indication for the abortion was her own will. An non-matured life is going to pay for his mothers and fathers wrong and quick decisions. The fetus was 6.5 weeks old. Then the catheters, brushes introduced in to the uterus through vagina and they did all the manipulations correctly and then the suction tube introduced and sucked all the bloody materials from the womb.   Then they sterilize the vagina, uterus, external genital area and administrated pain killers. Mother walked to the ward without any problem. Then the next mother came their with a great smile. she was even smiling and talkative with the doctors. Indications to the abortion was mothers will. I couldn't take this any more I came out. my friend a Indian guy came out he was just about to faint. we waited outside till the end. If you are so young be careful some decisions make us suffer later.

Stories which may have become love stories.

Charon is a boy. He had many problems some of them are battles than problems. About the love for asians as charon, there are limits and boundaries. Some times exceeding a boundary will be a great event. Also may be a  wining of an event. For many of situations there were happiness and joy beyond those boundaries. charon also had some boundaries. but they were fulfilled according to others decisions. But charon doesn't have a frustration about these winnings at the boundaries. Some boundaries were totally losses. But some of them gave a joy to his parents such as his exams ordinary level and  advanced level. When he got to be a talented student after my 11th class his parents were so happy. The day came my university entrance letter. They were all devoted and honored memories of charons life. Charon like to dedicate all those milestones in his life to his parents. He was a dedicated son but not always. some of the goals partially fulfilled by charon. So charon has many debts on his side. But his parents were ok with it. thats why charon always dedicate his life, victories and happiness to his bellowed parents. When charon around 16-17 year old he fel the love for the first time. but the family levels(I dont care about them) were not compatible, and her mother had a mental disorder,(I dont like to insult her). Her mother was successful at her first attempt of suicide. So no more talks. charon had to leave the idea. Even before she knows it. for the second time charon saw a girl in his advanced level classes but the times were not good. we were busy to make the exam success so charon lost the second chance. Then charon met a very nice Russian girl in Russia. charon didnt tell her about his love because there will be a big resistance from his parents. He didnt have a will to keep the both sides suffer. According to charons point of view a love should be a very beautiful thing. but to find a perfect love you should give up many things. charon did so. he doesn't have a will to see a tear on his parents eyes or her on her girlfriend's eyes. charon looks for his future. there also many boundaries. they are fine. but the limits. limits are the most difficult. Time, life, relationships, human values, love, ....all those things can be find in the limits of our lives. even life has its own limit. we dont know when will it end. The funny thing is yet we play on this unstable grounds. love life all become illusions.

Need of national drug policies.

for the first time in world history a drug controller poly is brought back in mid nineties. I would like to dedicate its credit to the Professor senaka Bibilie. Many dont know about this character but his vision was not only aimed the Sri Lankan people. The real effect which can be made by a fully legislated policy, could have make a big difference in today competitive drug market. According to the policy the drugs are should be in high standards. The quality of the drug is not be determined by the producer or the country which the drug is produced. With a equal quality of drug production there is no urge to write a brand name on a drug and put a extra expenses on the innocent patients. According to the plan the a drugs's generic name can be only used in pharmacies, clinics and in hospitals. So the drug producers are passively forced to print the generic drug name without printing a brand name on a drug. Then An every country can establish a Central Drug controlling agency (CDCA) by the agency can determined and control the drugs which the country produce and the drugs which import from another country. By this agency every country gets the power to distribute drugs only with generic name. So the generic named drug prices will be very low in price than the branded drugs. This policy completely aimed the people who are living in the developing countries. In these countries the annual income of a person is low. What Professor Bibile wanted to do is reduce the drug prices and ease the lord on the people. Governments took the responsibility to make the policy active. But the developing resistance from foreign drug companies through foreign governments and direct influences on the health sector by those companies made the project one of the weak projects. Even WHO has no clear view of this policy. Who doesn't define in which side are they working. (In the peoples side or in the private health provider's side ). In such a sad background of impossibilities of direct decision making by governments, national health policies and world health organization made the golden theory a useless one. But even for today we are not late to activate the bibile's drug policy and give benefits to people. This idea should be conceder and re conceder by the  authorities. In such a day these good ideas have a real place national health systems will be far better than the current health care facilities.

Definition to a perfect dream

In my life there were no many big dreams. many of them a really smaller than others dreams who were in the same age. I was always in aware about my family financial state. I can still remember my dream about a mountain bicycle. It became true when I was 15 or 16 years old.(True I had a small cycle/chopper). I told about my dream to my parents. but I never forced them to do it. I can still remember how determined them were about me. I have my own sister and my own brother. I love them. But the real meaning of a love to a brother or a sister  I understand now. At this moment im very far from my family so I know what a family really meaning. I understand how important is the family is. From my child hood there were no my requests to my family to buy toys(From me. im not talking about my brother/he is a good guy). When I was just 13 years old I could understand all the financial problems in my family so I blocked 90% of my dreams and locked them. I asked only about my education needs. the problems was that time my education was not a good one. A parent's meeting means a killing day for me. My teachers blamed to my parents because I was a weak student. I came to the Mahanama college when I was 7 years old. I left Malwana village school. Its not because I like to leave it, but pure desire of my beloved parents to give me a good education. Now as a 26 year old boy I think they were correct. They always love me. Mahanama is the place I learn about the society a lot. There were many cults. The cult of bright and English fluent is was the leading one.(This is till 5th class) I always respect my teachers but I should tell the were so delight to teach this language to the English fluent students. But to me an some others this was a hell of a 40 minutes. Because im from a village there no one speak english, and the group of near by area students also suffered with same problems. Teachers thought all of us comming from a one family. That was the biggest mistake. But after 5th class(Yea I failed from the scholarship examination). Failing the scholarship exam and the changes in this city school curricula kindles situations. Some times beatings by teachers because we were weak students made a depression in my mind. same repeat  and increased these kind of situations made some psychic traumas to me.(Once teacher once named me as a student without parents.) I keep my gratitude to Mahanama even between such negative conditions it was a nice shelter to my education. After 11th class many of the blames to my parents in the parents meetings stopped. That time I was a hard worker. I did my best to get in to the Medical college but I wasn't successful.  After my third attempt My parents sent to me to Russia. To have medical education. This part of the story ends now.
Another part starts. when I was doing my A/L exams My grand father left the world. One of the people who inspired me except my father. When I went to my vacation 3 years ago when I was saying good bye to return to Russia grand mother wished me and told me son you wont see me again. she was correct.
In a day im so old and going to die. Some one ask what kid of a dream would you like to see in my last days night this is my answer.-My grand mother and grand father infront of my house. as usually grand father walking or working in the garden. Im sitting in a bench between my father and mother. my brother and sister playing in the garden. grand mother and grand father in a discussion. its a evening. sky is pink and purple in color. And im going to the most sweetest sleep in my life. im so happy.

Memorizing heroes among students

A small story about some real sri lankan students who were in Kursk a long ago.
Wring this letter to reveal the real ability and the real fights for living. I don’t have any desire to insult anny sri lankan tamil student or a Sinhala student in this letter. This is a collection of their stories. There were main three characters in the story. As in my story too. The names are not real but the story line is a pure one.  Gananathan and Wasanthan were two tamil students who came to Kursk State Medical University around ten years ago. As usually their sri lankan contractors (People who sent them here) very clever.  Each student had paid 6000$ at the beginning. Their contractors told them this is the course fee. Gananathan Wasanthan paid this money individually. Gananathan had no parents. Their sri lankan contractors lied to them. They thought 6000$ is the last payment to do to be a doctor. The nice thing was they were convinced by some cheaters to believe their course fee as 6000$.
They came to Kursk in a nice winter time and started their studies here.  The real face of the international education met them after one year. Gananathan and Wasanthan were astonished when the university (Yes the same university KSMU, our Alma Mater) asked money for the further education. They became mad. Time passed little by little. They tied to earn money .They new what are the hunger pains means. They knew how hard to live 3 days with few kopeks in their pockets. But the determined nice fellows started the fight against the cruel environment. Gananathan started to cook and sail fast food, Wasanthan was a little weak but he was in the fight. Wasanthan later took a decision to marry a Russian girl because this may give some relief to his crucial problems. Think about a person without food. As the writer I have felt it. Some of my cooking mates now these situations.  They felt the same pains but not long like Wasanthan.
Wasanthan one day went to one friend’s house just for a visit. It was a day time.  Friend Asked “hey brother do you need something to eat? “Did you take your lunch?” Wasanthan told the truth. They were starving for two days the only food was Russian Black bread.(I never forget this food for my life time). His friend became so sad. He cooked and fed Wasanthan and asked about the problem. The girl having her menses he doesn’t have buy the utilities. The friend bought the necessary things from the shop and he bought a chicken and rice too. Wasanthan was so thankful to the friend. As the writer I really respect that Russian girl who took such difficulties in their marriage life. Today such girls are very rare. Even Russians will prove my pint. But the life of wasanthan  was filled with many difficulties  he couldn’t earn enough to pay the tuition fees. That time the immigration rules were not so strict as today. Wasanthan escaped to France with her girl friend. One day he called that Sri lankan student who helped him when he was in a critical condition.  He told”Brother I came to France with my wife. We have jobs now brother if you need some help tell me I will send money”. Jananathan ended his medical education and started a new life. He didn’t forget the people helped him. But a student came to be a doctor change his way due to financial problems, helplessness and strict and cruel administration of the university. I stop with the wasanthan’s story now. Gananathan was a bright student. He studied cooked, delivered them, and worked as a waiter in a Sri Lankan café. He earned as much as he can but it wasn’t sufficient to pay the course fee on time. Gananathan also said good bye to medical education later. When he was in second year Gananathan decided to stop his education and face his destiny. He didn’t had his parents even. Se the cruel attitudes of the society, cruel characteristics of the educational institutions and hard social conditions changed their way. But they never defeated. Some news from Canada and france Germany confirm that they are spending a good time there. In my memory there were many like this from our own country. Im not insulting them. I consider them as heroes in my time.  Getup at 2 am cooking, delivering, selling products even in the lecture halls, 1.5km walk to buy  several bundles of Cocacola, Pepsi, Milk and etc to keep a small profit(our third character Menaka). A profit not spent to a beer bottle. Hardly earned profit for education. When the professors throw their students (their children) out from the classes these heroes dedicated their enjoyable student life nice chats, material discussions, romances, sports and everything what they had as a university student.  Hey you! You don’t have your class permission go out? We go out from the class. We blame no one. I think in another way these make real doctors.  I do not appreciating my administration.  They could have made a big difference but they don’t do it. Always such changes start from the small people like us specially students, the level of people who really suffer because of useless rules. Students should learn well. Some day there me be a wave of change. Then most of us can join with it and can support to the real fight of Gananathan’s  wasanthan’s and many others which I didn’t mentioned here. French revolution is a good example.
